Schedule: Monday-Friday - 8hr shifts or 4/10s schedule
Responsibilities:
- Patient Volume: Averages 12-15 per day
- Staffing Mix: 2 full time PTs, 1 PT Aide and 1 Front Office staff member
- Assess patients and develop individualized Physical Therapy treatment plans
- Provide treatment interventions for outpatient, general, and surgical patients
- Develop discharge plans and provide patient education
- Teach therapeutic exercises and movements to improve function and prevent or reduce recurrence of injury
- Educate patients regarding physical limitations following surgery, injury, or impairment
- Perform and interpret tests and measurements to evaluate patient condition and treatment progress
- Modify treatment plans, protocols, procedures, and equipment as clinically appropriate
- Document patient progress in the electronic health record in accordance with AAAHC and CMS documentation and coding standards
- Provide Physical Therapy consultation and evaluation services to physicians, providers, nursing staff, and other healthcare professionals
- Communicate with physicians, nursing staff, and other members of the care team regarding patient condition and recommendations for continued care
- Provide direct supervision to support staff, volunteers, and Physical Therapy students as indicated
- Participate in patient, staff, and community education
- Coordinate with federal, tribal, private, and community resources as appropriate
- Maintain patient confidentiality and adhere to professional, ethical, organizational, and safety standards
Position Requirements:
- Doctor of Physical Therapy (DPT) from an accredited program, or Master of Physical Therapy (MPT) or equivalent along with required experience
- DPT preferred to have minimum 3-years experience and MPT or equivalent preferred to have 6-years of licensed PT experience
- Current, valid, unrestricted Arizona Physical Therapist license required, prior to start (will accept compact license)
- USPHS Commissioned Officers may hold a current, valid, unrestricted PT license in any US state, District of Columbia, or Puerto Rico
- APTA Board Certification preferred
- Valid, unrestricted, insurable driver's license
- Active BLS Certification
- Comfortable treating primarily adult patients and occasionally patients under 18
- English/Navajo bilingual skills preferred
