NPDB Self-Query

Please follow the instructions below to obtain a NPDB Self-Query search in the database. It will cost $3.00 and should take about 10 minutes to complete.

1. Visit the website: https://www.npdb.hrsa.gov/pract/selfQueryBasics.jsp
2. Sign in to or create an ID.me account
3. The type of NPDB search is: Personal
4. Go to Step 1 – Please fill out as much information as possible – this is some of the information you will need on hand:
   1. Prof. License #:
   2. NPI:
   3. DEA:
5. Go to Step 2 – Fill out payment information
6. Go to Step 3 – Review all information
7. Go to Step 4 – It will have you verify your identity with some financial questions
8. Submit!
