Physician License Application
Fees
Total of $400 when you apply online
Uniform Application (UA)
To use the UA, visit http://www.fsmb.org/ and select Uniform Application (UA) from the Licensure or Sign In menu. Sign in and continue as directed.
Additional Documents Needed
- Federation Credentials Verification Service (FCVS)
- Follow this link to begin the process or request an already completed FCVS profile be sent electronically to the WYBOM.
- Three physician colleauge references (Send them this link to fill out the questionnaire).
Temporary License
All physicians are automatically eligible for the temporary license that will be granted in anticipation of the next WYBOM quarterly meeting where permanent licensure is granted.
License Processing Times
Processing time for licensure depends on a few factors. If you are currently licensed in an IMLC compact state, licensure will be granted within 2 weeks. If you are not in an IMLC compact state and have never used the FCVS before, your license will be granted in about 2 months.
Other Critical Steps
DEA Registration
You will need a Wyoming address on your DEA license. This is not a prerequisite for your physician license to be granted, but you will need to register your DEA license in Wyoming prior to starting work in the state in order to prescribe legally.
There are two ways to ensure you have a DEA license registered in the state you will work in. Some providers decide to purchase an additional DEA license to devote to locums work, but most choose to simply transfer their existing DEA license to Wyoming immediately prior to their assignment starting. Whichever path you choose, you can apply for a new license or make changes to your existing DEA registration at the DEA website.
Keep in mind if you choose to transfer your existing DEA license to a new state, the process can take up to 5 business days so you must account for that transition time.

You will need to apply for the CSR in order to have full prescribing privileges in Wyoming.

Notary and Photograph
Keep in mind, as you apply for your Uniform Application you will have to mail your application in and include a notarized page with a 2″x2″ passport-style color photograph of yourself.
